Going full online, as in an MMO would completely change the game to something that isn't Elder Scrolls. One of the key aspects of it is character interaction, but if some idiot kills off all the NPCs, then WTF are you gonna do? Elder Scrolls universe responds to the player where an MMO can't do that because it has to be equal and fair to everyone. There is also very minimal character protection in ES, so pros could completely pwn noobs constantly with no repercussions, and if there were consequences, that would ruin the game as well.
However, privately hosted servers would be great in my opinion. Hosting your own "realm" only letting your chosen friends in would be pretty awesome, especially if you could import your characters to friend's games. Of coarse you would have to operate from the hosts save game, where ever in the game's storyline that may be. The best thing to do would probably be to have a save game profile with all of your friends together to play on when you can, and other individual profiles on their own, because it wouldn't be to fun if everyone were different levels and at different points.
