Make sure everytime you reload its an active. Don't rush it either, stay back as much as possible, and stay in cover and kill people patiently.
Hardest bit for me on insane was in Act 3, when you're going through the mines on the cart and theres the exploding Kryll.

I beat insane in 4 hours with a friend. The WHOLE campaign.

I found the hardest part was in the mines, the room before you get in the carts where you have loads of exploding Kryll coming at you from all directions. Took me ages to get through that part!
With the rest of it, be patient, and take lots of cover!!
