I didn't bother to watch the credits but i still got it. Are you sure you beat all the missions? Maybe theres something left check your map.

OMG YES I HAVE DONE ALL 94 MISSIONS I HAVE CHECKED MY MAP I HAVE KILLED THE LAST TWO PEOPLE YOU KILL. Sorry just getting annoyed with people asking have you done all the missions and u sure u didnt cheat.....

Its probably just a glitch. Look at the Rolled Over achievement. Took me 2 fuckin hours to get it.
