For me the scariest game I ever played was Fatal Frame on the PS2. That game really scared the hell out of me. Something about female ghosts floating backwards at you with their head snapped backwards really freaks me out lol.

For me, it's gotta be, the first Res Evil on the PS.
Me, my brother and our mates took it in turns to play - 15 minutes each and it would get to the stage where we'd not want to take our turns LMAO!

The first, oh... maybe 10-12 times the lights suddenly go out in the room and monsters literally crawl out of the walls in Doom 3, it's pretty suspenseful.
The next 500 times after that, it just got a little tedious.

Condemned made me jump a lot and come close to having a heart attack. Fatal Frame and Forbidden Siren both freaked me out a lot on the PS2, but I was a lot younger when I was playing them.
Silent Hill on the PS1 was undoubtedly the scariest experience I've ever had though. I was only small at the time and I distinctly remember nearly wetting my pants on several occasions. Most of the time I just used to watch my older brother play, but even then it was terrifying.

I'd say the first or second Resident Evil, I can't remember which one, on the PlayStation. Scary as hell.
