Longer main quest. (Y)
More diverse set of faction quests, instead of just do quest, run back, get reward, do next quest. (Y)
And yeah, the combat could do with some tweaking...

A way to mod the game on consoles with an online user marketplace to share mods. Bigger world with more transportation options. More use of water throughout the game. More guilds and when you beat the guild's storyline you should be in control of it, making decisions, starting tasks, alliances with other guilds, etc.

and people should react to you a lot differently when youve done certain things like beaten the main quest.

They need to clean out as many glitches as possible.
Many a quest has been ruined by them

I think it would be a good idea to have up to 4 people on co op mode online like halo did but not make the game into an MMO. Also have a longer main quest with maybe a few more cut scenes, further more have xbl constantly update the the game with new quests without having to download and pay for them off xbl. Like have contest where a random joe comes up with a new plot for a quest and emails bethesda and they would in tern take the guys idea and put it in the game, and do this monthly for like 1 year.

Big epic battles
Online Arena (1v1, 2v2, 4v4 etc)
Be able to lead massive armys around the map, and lay seige to any citys (ties in with the epic battle I spose)
I could go on and on..
