I'm not sure whats wrong. However you can try tightening the xclamp screws and then heat it up the way llama says. Whats the secondary code?

Let me google that for you <SECONDARY ERROR CODE

If it has been 5 months since your fix, that means your screws have become loose due to heat cycles (expansion / contraction).

Tighten them in a star pattern (like what you should do on your tires / engine block)

Can't explain

I have had this and have fixed it. The problem is i am very bad at explaining things sometimes. try hacking it to look more clearly for the problem.
(need computer with proffessional interface and enough USB cables for the xbox's 3 inputs)

I am not a proffesional.
my advice is not 100% advisable.
any errors or breakage is not my fault. (But if it is... don't try it please)

another thing... my advice isn't even 5% advisable either so do so at your own risk.
